DOT Violations/Defect Code 4
Defect Code 4

Hardware Defect (Covers, Gratings, Vault Lights)

Hardware that sits proud of, sunk below, or rocks within the sidewalk plane is a citable DOT defect. We reset frames in fresh concrete so covers and gratings sit flush and stable.

Signs You Have This Violation

  • Manhole cover rocks or rattles when stepped on
  • Grating sits below grade and traps water
  • Vault light glass cracked or missing
  • Frame separated from surrounding concrete

Repair Steps

  1. 1
    Identify the utility
Con Ed, National Grid, NYC DEP — we coordinate with the owner if needed.
  2. 2
    Saw-cut around frame
Demo concrete around the frame to free it for resetting.
  3. 3
    Reset to grade
Frame leveled flush and locked in with non-shrink grout.
  4. 4
    Pour collar
New 4" concrete collar tied to surrounding flags.

Typical Price Range

Range
700 – 2,200 per fixture

Final price depends on access, number of flags, and whether a DOT permit is already on file. Every job includes a free written estimate.

Frequently Asked Questions

Aren't utilities responsible for their own hardware?+

Often yes — but DOT still cites the property owner. We handle the repair and you can pursue the utility for reimbursement separately.

Will the new collar match?+

We score and finish the collar to blend with adjacent flags as closely as possible.
